Hospital, not just extras
Extras-only policies for dental, glasses and physio do not remove MLS. A Basic hospital policy may qualify, but check the actual policy: Basic cover can have very limited treatments.

AUSSIE HEALTH COVER SAVINGS CALCULATOR / AUSTRALIA-WIDE / 2026–27
Use Aussie Health Cover Savings Calculator — an independent Australian private health insurance cost tool. Estimate your Medicare Levy Surcharge and compare it with the price of eligible hospital cover.
For all of Australia: MLS is a federal tax, so the same surcharge thresholds apply in NSW, Victoria, Queensland, WA, SA, Tasmania, ACT and NT. Insurance prices and some benefits vary by location.
The Australian Government uses the Medicare Levy Surcharge (MLS) to encourage higher-income people to take out private hospital insurance. If your income is above the relevant threshold and you don't have qualifying cover, you may pay extra tax. Sometimes a qualifying hospital policy costs less than the MLS it could help you avoid.
This project started with a simple lesson: compare the numbers before assuming private insurance is only an added cost. You may discover a lower annual premium and some hospital benefits. Or you may find that insurance isn't the right option for you.
Why MLS exists — ATO explanationNumbers are approximate, use general 2026–27 rules and do not account for every exemption or policy condition. Do not rely on this calculator alone to buy insurance or make tax decisions. Confirm your MLS position with a registered tax agent and ask the insurer to verify policy eligibility, the final premium, hospital exclusions and your Lifetime Health Cover loading. Read the limits and privacy information.
Extras-only policies for dental, glasses and physio do not remove MLS. A Basic hospital policy may qualify, but check the actual policy: Basic cover can have very limited treatments.
The eligible yearly front-end excess is generally no more than $750 single or $1,500 couple/family. All required family members must have eligible cover.
There's no universal minimum dollar premium. Compare a real all-in annual quote (rebates + any age loading) against your estimated MLS. Cover only counts from the insurer-confirmed start date. Buying just before lodging tax does not erase previous uncovered days.
The ordinary Medicare levy generally still applies. Even an MLS-eligible policy can exclude many treatments and leave you with excess, co-payments or doctor gaps. Government MLS eligibility rules ↗

Some lower-priced Basic hospital policies can satisfy the insurance requirement for the Medicare Levy Surcharge (MLS) — but may pay very little towards actual hospital treatment. These are historical price examples, not live quotes or recommendations.

Important: The prices shown are single-adult examples only, as published for 2026 (cross-checked against the October 2026 data). They are not live market rates, a guaranteed cheapest list, or whole-family quotes. A government rebate, age-based discounts, Lifetime Health Cover loading, postcode, insurer changes and membership rules can alter your price. Cheap Basic cover often excludes most hospital treatment categories and may leave substantial out-of-pocket costs. An MLS-eligible policy only avoids the surcharge for eligible covered days; it does not cancel any MLS for earlier uninsured days.

The surcharge is assessed for the financial year, 1 July to 30 June — not the date you lodge your tax return. To avoid MLS for the whole year through insurance, everyone who needs cover generally must hold qualifying hospital cover for every day of that year.
There isn't a fixed minimum number of months of cover for partial-year MLS relief: each qualifying covered day generally counts. Hospital-treatment waiting periods are a separate question from the policy's effective date. Confirm that date, dependants and eligibility with your insurer. Official rules ↗

A married couple uses the family MLS thresholds even with no children.
Enter taxable income , not turnover. A sole trader uses business profit after allowable deductions, plus other personal taxable income.
Only enter amounts NOT already included in taxable income. These can change the tax tier without always changing the amount the MLS is charged on. This section follows the ATO income definition; seek a tax-agent check for unusual income.
LHC (Lifetime Health Cover) is an extra amount added to the hospital premium , not a separate tax. It isn't the MLS.
You can compare policies from different insurers. This calculator does not invent or download live premiums.
No insurer prices are preloaded. Enter an actual insurer quote and confirm its final premium and MLS eligibility. Policies may have treatment exclusions, restrictions, co-payments, doctor gaps and waiting periods even if they avoid MLS.
Extras are separate from hospital cover. Estimate what the insurer would actually reimburse for your household, not the full price of treatment. Check benefit limits before entering amounts.
Compare the extra tax avoided with the net price of insurance . Benefits, exclusions and gap payments still matter.
Additional tax only; the standard Medicare levy is unchanged.
An eligible policy only removes MLS for covered days.
These are your final checks. The lowest premium can still come with major exclusions or out-of-pocket hospital costs.
Designed for salaried employees, sole traders and households. No accounts, uploads or financial documents needed.

We'll estimate your annual Medicare Levy Surcharge using published 2026–27 thresholds.
If an eligible hospital policy costs less than the surcharge, you may save money and gain cover.
An independent Australia-wide educational estimate under general 2026–27 MLS rules, created with AI assistance. Not an ATO product, a financial-product recommendation, or a live insurer-quote service. Some exemptions and income adjustments require professional review. Buying hospital cover does not remove the ordinary Medicare levy. Do not rely on this tool alone when deciding on insurance.
This project makes a complicated tax rule easier to explore. It is not a substitute for a personalised review.
The calculator estimates the 2026–27 Australian financial year using your inputs and publicly available federal rules; NSW has no separate MLS rate. Insurance quotes and some benefits depend on your state and postcode. Partial-year amounts are pro-rata illustrations, not an exact assessment. It assumes the household setup and income are stable, unless the detailed mode says otherwise. A hospital premium below MLS is a numerical comparison, not proof of overall savings or suitability. Real costs may include excess, gap fees, exclusions, waiting periods, rebates, Lifetime Health Cover loading and part-year adjustments.
